The acceptance of this policy is mandatory for access to the Dakota Valley School Network & Network Privileges

Student Computer Insurance is Optional for the cost of $25.00 per year. Paperwork MUST be signed and turned in with NO Insurance or Personal Insurance or School District Protection CLEARLY marked on form

This form must be signed by a parent annually in order for a student to receive Tylenol or Ibuprofen during the school day annually

This form must be signed by a physician and parent if a student is to receive prescription medication
